Pre-Prescription Form
- Patients who are approved will receive a Pre-Prescription Form confirming that a consultation was completed and that they were approved for medical cannabis.
- It lists the doctor's name and the date.
- It lists the recommended CBD and THC products.
- It states the method of consumption advised.

- Certification involves intensive, evidence-based training — roughly 201 hours of coursework built on more than 350 scientific citations.
- Training covers dosing, CBD and THC product selection, methods of consumption, drug interactions, patient monitoring and the requirements of Law 10113.
Option to fulfil through Farmacia Azul
- You may choose to have your prescription filled through Farmacia Azul, a pharmacy partner of Cura Vida and MEDEC-CR that fulfils and distributes digital cannabis prescriptions nationwide.
- Products dispensed are pharmaceutical-grade and lab-tested.
- Where you fill your prescription remains your choice.

- Medication is purchased separately at the pharmacy and is not included in the package price.
